STJSTMM Festival — Liability Waiver and Release of Claims

By signing this waiver, I acknowledge and agree to the following terms and conditions for participation in the STJSTMM Festival activities, including but not limited to soccer, bounce houses, games, and all other physical activities:

1. Assumption of Risk: I understand that participation in physical activities involves inherent risks of injury, including but not limited to sprains, fractures, or other physical harm. I voluntarily assume all such risks.

2. Release of Liability: I hereby release, waive, and discharge STJSTMM Church, its clergy, volunteers, organizers, and all affiliated parties from any and all claims, demands, losses, or damages arising from my participation in festival activities.

3. Medical Authorization: In the event of an emergency, I authorize festival staff to seek appropriate medical attention on my behalf (or my child’s behalf if signing for a minor).

4. Rules Compliance: I agree to follow all rules and instructions provided by festival staff and volunteers. I understand that failure to comply may result in removal from the activity.

5. Minor Participants: If signing on behalf of a minor, I confirm I am the parent or legal guardian and have the authority to sign this waiver on their behalf.
